Bleomycin 5 mg/d IM days 1-5 during the first week and days 1, 4 during subsequent 5 weeks CCNU 40 mg/d PO days 5-7 Methotrexate 15 mg/d PO days 1, 4 during the first week and day 1 during subsequent 5 weeks Repeat 6-week cycle every 49 days Appropriate premedications listed in Section 2, Commonly Used Premedication Regimens
